The following documents will be required to complete your family’s application:
- Enrollment Application
- Child’s birth certificate or crib card
- Child’s current immunization record
- Proof of address (must reside in Douglas county)
- Proof of income for 12 months (1040, W2, pay stubs for the last year, TANF, or SSI statements)
If applicable, please also provide:
- Documentation of Title XX
- Documentation if your child has an identified disability (IEP or IFSP)
- Foster care placement paperwork
- School financial aid and school schedules
- Child support information
